On July 14, 2000, during the renovation of a cottage in Lijialing, a construction company experienced a tragic accident. A migrant worker operating a mortar machine was electrocuted due to a faulty electrical system, resulting in his death. The incident occurred when the worker was using a 7.2-meter-long steel pipe to pour water into the mortar mixer. After mixing, he attempted to transfer the mortar but was suddenly shocked and fell to the ground. At around 1:30 PM, the worker, Yang Moumou, was handling the mortar machine when he accidentally placed the iron pipe on the motor terminal box. Due to a loose connection, the metal casing became energized, leading to the fatal electrocution. A nearby worker quickly cut off the power, but it was too late—Yang had already passed away. Investigations revealed that the direct cause of the accident was a poorly maintained mortar machine. The motor’s terminal box had only one screw securing the cover, which was loose. When the iron pipe was placed on the box, it caused the cover to shift, allowing contact between the exposed phase terminal and the metal casing. This led to the entire machine becoming live. Indirect causes included the failure to disconnect the main power supply before operation, improper grounding of the neutral line, and the absence of a leakage protection device. Additionally, the site lacked proper safety measures, such as insulated tools and protective gear. Yang was wearing wet shoes, and the ground was moist, increasing the risk of electric shock. The investigation also highlighted poor management practices. Safety inspections were not properly conducted, and many workers lacked proper training or certifications. The equipment used was not up to standard, and there was no effective supervision or maintenance protocol in place. These factors contributed to the unsafe working environment that ultimately led to the tragedy. This incident underscores the importance of strict adherence to electrical safety standards, proper equipment maintenance, and comprehensive worker training.
